Husky Handbook Enrollment & Change Forms

• If there are No Changes from the previous years Husky Handbook, we will assume the information is the same and will continue to include the information in the 2007 Husky Handbook.

• Only use the New Enrollment Form if you would like to be included in the Husky Handbook for the first time. This includes new kindergarteners and new students to the school as well as families that now wish to have their information listed in the Husky Handbook.

• Use the Change Form if the information in the current Husky Handbook is not correct or has changed. You do not need to send in a form with the student’s class assignment. The school will provide all class information in September.

•  Online payment for the Husky Handbook can be done from the Online Membership Form.

• Deadline for Changes is August 15, 2008.

• Deadline for New Enrollment is September 5, 2008.
